Creatinine, with the chemical formula CHNO and CAS number 60-27-5, is a nitrogen-containing organic compound. It appears as prismatic crystals at room temperature, with a melting point of 230-240°C (decomposition), and a density of 1.474 g/cm³. It is soluble in water, slightly soluble in ethanol, and almost insoluble in ether and acetone [1] [5]. Its alternative names include 2-amino-1-methylimidazoline-4-one and creatin [4-5]. It is mainly produced by heating hydrated creatine with hydrochloric acid, followed by precipitation with ammonia water, crystallization, filtration, and drying processes.

Improving respiratory function
NAC can stimulate the secretion and formation of surfactant by type II cells in the alveoli, enabling the alveoli to expand effectively and increasing the area for gas exchange. It achieves this by activating anti-protease and inhibiting the elastase released by polymorphonuclear neutrophils that damages alveolar elastin. A group of studies have shown that compared with the non-treatment group, the annual average attenuation of FEV1 in the treatment group using NAC was significantly lower.
Antioxidant effect
The free radicals derived from oxidative processes play an important role in the pathophysiology of the lungs. Smoking or the release of endogenous substances from inflammatory cells (neutrophils or macrophages) can lead to an increase in free radicals. The inflammatory response in the lungs can produce a large amount of oxygen free radicals and elastase, which destroy the activity of α1-proteinase inhibitor and cause damage to lung tissue, resulting in the destruction of alveolar structure. Free radicals promote the development of emphysema by affecting the balance between elastase and anti-elastase. NAC can inhibit the replication of viruses, stimulate the replication response of T-lymphocytes, increase the number and activity of T-lymphocyte subsets, increase the adhesion and phagocytic function of macrophages, protect cells from damage caused by viral infection, and thus play a role in preventing and reducing respiratory infections.
Reducing the number of bacteria in the airways
The increase in the deterioration rate of patients with chronic bronchitis may be related to the change in the bacterial flora in the bronchi or immune deficiency. A study (EurRespir J. 1994;7;94-101) using bronchoscopy and a protective sputum brush to collect lower respiratory tract sputum for bacterial culture showed that in 21 patients in the non-NAC treatment group, 15 types of bacteria were isolated, while in 16 patients treated with NAC, only 3 types of bacteria were isolated. This provides a reasonable explanation for the reduction in the deterioration rate of chronic bronchitis patients taking NAC.
